ABSTRACT

ABSTRACT Ocular cysticercosis is a parasitic infection caused by Taenia solium. Its early diagnosis and treatment decreases the possibility of visual morbidity. It can either compromise the anterior chamber or the posterior segment, which translates into an very variable and interspecific presentation that changes depending on the site of the infection. It is important to report this case due to its low presentation rate and the fact that a high suspicion index is required to make an assertive and timely diagnosis. This is especially important in geographical areas that are endemic to this parasite due to the direct relationship between an early diagnosis and treatment and better visual outcomes. In this case report, we will discuss the multidisciplinary interventions of a pediatric patient in a high complexity hospital.


RESUMO A cisticercose ocular é uma infecção parasitária causada pela Taenia solium. O diagnóstico e tratamento precoces diminuem a possibilidade de morbidade visual. Ela pode comprometer a câmara anterior ou o segmento posterior, o que se traduz em uma apresentação muito variável e interespecífica, que muda dependendo do local da infecção. É importante relatar esse caso devido à sua baixa taxa de apresentação e ao fato de que é necessário um alto índice de suspeita para fazer um diagnóstico assertivo e oportuno. Isso é especialmente importante em áreas geográficas endêmicas para esse parasita, devido à relação direta entre diagnóstico e tratamento precoces e melhores resultados visuais. Neste relato de caso, discutiremos as intervenções multidisciplinares de um paciente pediátrico em um hospital de alta complexidade.

Resumo Objetivo: A queratite infeciosa é uma doença de incidência relativamente elevada e é responsável por um número importante de internamentos. Neste estudo pretende-se estudar diversas características epidemiológicas e clínicas associadas às queratites infeciosas de alto risco num hospital terciário em Portugal. Métodos: Realizou-se um estudo retrospetivo, onde foram incluídos todos os doentes internados por abcesso da córnea no Centro Hospitalar Universitário do Porto (CHUP), entre Abril de 2013 a Março de 2018. Caracterizou-se a população em relação aos fatores de risco, apresentação clínica, tempo de internamento, resultados de culturas, resistência antibiótica in vitro, tratamento efetuado e resultado funcional. Resultados: O estudo incluiu 105 doentes. Os principais fatores de risco foram antecedentes de cirurgia de córnea, uso de lentes de contacto e história recente de trauma ocular. 74,3% dos doentes tiveram cultura positiva com 87,9% a corresponderem a cultura bacteriana pura, sendo a Pseudomonas aeruginosa e o Streptococcus pneumoniae os agentes etiológicos mais frequentes. 27,9% das culturas positivas eram resistentes a 3 ou mais classes de antibióticos. Todos os doentes iniciaram tratamento com colírios fortificados. 29,5% dos doentes necessitaram de realizar transplante de córnea. Ao final de 6 meses de seguimento, apenas 20,9% apresentavam AV>20/40. Conclusão: Na maioria dos casos, a etiologia foi bacteriana. Observou-se um número considerável de bactérias multirresistentes. Apesar do tratamento ter permitido uma melhoria da visão na maioria dos casos, um número considerável de doentes ficou com sequelas visuais importantes.


Abstract Objective: Infectious keratitis is a pathology with a high incidence and is responsible for a large number of prolonged stay hospital admissions. The purpose was to analyze the epidemiological and clinical data associated with high risk microbial keratitis at a central hospital in Portugal. Methods: A retrospective study of all inpatients presenting with corneal abscess in Centro Hospitalar Universitário do Porto, from April 2013 to March 2018 was performed. Target population was characterized by risk factors, clinical features, length of stay, culture results, in vitro antibiotic resistance, treatment and outcome. Results: This study included 105 patients. The main risk factors were previous corneal surgery, contact lenses wear and recent history of ocular trauma. 74.3% of patients had a positive culture, 87.9% of these corresponding to a pure bacterial culture, with Pseudomonas aeruginosa and Streptococcus pneumoniae being the most common pathogens. 27.9% of positive cultures were resistant to 3 or more classes of antibiotics. All patients began treatment with fortified drops. 29.5% of patients required a corneal transplant. After 6 months of follow-up, only 20.9% presented a VA>20/40. Conclusion: Most cases were caused by bacteria. A considerable number of multi-resistant bacteria was identified. Despite most cases having improved after treatment, a large number of patients had a significant visual acuity sequelae.

Resumen La oftalmomiasis es la infestación ocular por larvas de dípteros. Ha sido descrita en pacientes provenientes de áreas rurales, higiene deficiente o por contacto con animales. Sin embargo, existen reportes esporádicos de esta infestación en áreas urbanas. Presentamos el caso de un varón joven, sin factores de riesgo, proveniente de un área urbana al sur del Perú, que presentó 10 larvas en la conjuntiva ocular. Las larvas fueron identificadas como Oestrus ovis, una mosca que suele infestar ovejas y cabras como parte de su ciclo de vida. Realizamos una breve revisión del tema, resaltando la importancia de identificar adecuadamente la especie causante de la miasis, así como su ciclo de vida.


Ophthalmomyiasis is the ocular infestation by dipterous larvae. It has been described in patients from rural areas, poor hygiene, or contact with animals. However, there are sporadic reports of this infestation in urban areas. We present the case of a young man without risk factors, from an urban area in southern Peru, in whom we found 10 larvae at ocular conjunctiva. The larvae were identified as Oestrus ovis, a fly that usually infests sheep and goats as part of their life cycle. We made a brief review of the case, highlighting the importance of adequately identifying the species that cause myiasis as well as its life cycle.

ABSTRACT

The Global Burden of Disease Study 2010 listed schistosomiasis among the leading 100 causes of death in Brazil, responsible for 3.6% of the estimated total of deaths globally. Eye and adnexa are very rarely affected by schistosomiasis mansoni, with limited documentation of ocular pathology in this setting. This short communication reports ocular histolopathological findings in a murine model of neuroschistosomiasis mansoni. Lesions were found in the bulbar conjunctiva, lacrimal gland, choroid and corneoscleral limbus.

RESUMEN Se presentó un caso clínico de un paciente de seis años de edad, con toxocariasis ocular. La manifestación clínica fue disminución de la agudeza visual monocular. El fondo de ojo izquierdo mostró un granuloma retiniano periférico, asociado a un desgarro retiniano perilesional. El examen oftalmológico confirmó el diagnóstico de Toxocara ocular. El paciente fue tratado posteriormente mediante corticoides sistémicos y fotocoagulación láser focal en el desgarro retiniano (AU).


ABSTRACT We present the clinical case of a male patient, aged 6 years, with ocular toxocariasis. The clinical manifestation was monocular visual acuity decrease. The left ocular fundus showed a peripheral retina granuloma, associated to a perilesional retinal tear. The ophthalmological examination confirmed the diagnosis of ocular Toxocara. After that the patient was treated using systemic corticoids and focal laser coagulation in the retinal tear (AU).

RESUMO O objetivo deste resumo é relatar um caso de portador de oftalmomiíase externa, discorrendo sobre o quadro clínico, os diagnósticos diferenciais e as opções de tratamento. As informações foram obtidas por meio de revisão do prontuário, entrevista com o paciente e registro fotográfico dos métodos diagnósticos e terapêuticos aos quais o paciente foi submetido. Dados foram analisados junto a uma extensa revisão da literatura. O nosso artigo relata um caso de um paciente que foi inicialmente diagnosticado e tratado como celulite pré -septal e após avaliação de especialista em oculoplástica foi realizado o diagnóstico e tratamento adequado para oftalmomiíase. Também revela a importância deste diagnóstico, infrequente nos grandes centros urbanos, seu tratamento e evolução.


ABSTRACT The purpose of this report is to describe a case of external ophthalmomyiasis, discussing the clinical picture, differential diagnoses and treatment options. The information was obtained by means of a review of the medical record, an interview with the patient and a photographic record of the diagnostic and therapeutic methods to which the patient was submitted. Data were analyzed together with an extensive review of the literature. Our article reports a case of a patient who was initially diagnosed and treated for pre-septal cellulitis and after evaluation by a specialist in oculoplastics, the diagnosis and appropriate treatment for ophthalmomyiasis was performed. It also reveals the importance of this diagnosis, infrequent in large urban centers, its treatment and evolution.

ABSTRACT

La toxocariasis es una zoonosis parasitaria. Se describen las alteraciones anatómicas y funcionales producidas por membranas que traccionan el polo posterior y la periferia de la retina, las cuales provocan hemorragia vítrea parcial asociada a neovascularización del disco óptico con membrana epirretiniana, y un agujero macular lamelar relacionado con la presencia de granuloma periférico. Se presenta un caso clínico de un paciente sano, quien comenzó con disminución de la agudeza visual del ojo izquierdo y se le diagnosticó, por cuadro clínico y examen oftalmológico con resultado positivo de la prueba de Toxocara, una toxocariasis ocular complicada. Se le realizó cirugía de vitrectomía mínimamente invasiva 23 G asociada a membranectomía, y pelado de la membrana limitante interna, así como endofotocoagulación con láser panretiniana. Se usó como tamponador al final de la cirugía el gas SF6 y el posicionamiento del paciente. Se aplicó tratamiento antiparasitario y esteroideo sistémico previo a la cirugía, y se obtuvieron resultados satisfactorios tanto anatómicos como funcionales del paciente(AU)


Toxocariasis is a parasitic zoonosis. A description is provided of the anatomical and functional alterations produced by membranes that pull the posterior pole and the periphery of the retina, causing partial vitreous hemorrhage associated to neovascularization of the optic disk with epiretinal membrane, and a lamellar macular hole related to the presence of peripheral granuloma. A clinical case is presented of a healthy male patient who started out with a decrease in the visual acuity of his left eye and was diagnosed with a complicated ocular toxocariasis based on his clinical status and ophthalmological examination with a positive toxocara test. Minimally invasive vitrectomy was performed: 23G associated to membranectomy and internal limiting membrane peeling, as well as panretinal laser endophotocoagulation, using patient positioning and gas (SF6) as tamponade at the end of surgery. Systemic steroid and antiparasitic treatment was provided before surgery, and satisfactory anatomical and functional results were obtained(AU)

ABSTRACT

Abstract This study reports the occurrence of Austrodiplostomum compactum metacercariae in the freshwater fish Schizodon intermedius (ximborê/piava) from a Neotropical reservoir in the Paranapanema River, state of São Paulo, Brazil. From a total of 75 fish collected, we found 38 infected with A. compactum metacercariae (prevalence = 50.67%) in the eyes. The mean intensity of infection and mean abundance were 9.05 ± 4.26 (1-155) and 4.59 ± 2.20 (0-155), respectively. Poulin's Discrepancy Index was 0.87 and four specimens presented more than 10 metacercariae in their eyes. No correlation was observed between parasite abundance and standard length, total weight and condition factor (p>0.05). These metacercariae are reported for the first time in S. intermedius.


Resumo Este estudo reporta a ocorrência de metacercárias de Austrodiplostomum compactum na espécie de peixe de água doce Schizodon intermedius (ximborê/piava), oriunda de um reservatório Neotropical no rio Paranapanema, Estado de São Paulo, Brasil. Um total de 75 peixes foram coletados, sendo que 38 estavam infectados com metacercárias de A. compactum (prevalência = 50,67%) nos olhos. A intensidade média de infecção e abundância média foram 9,05 ± 4,26 (1-155) e 4,59 ± 2,20 (0-155), respectivamente. O Índice de Discrepância de Poulin foi de 0,87, e quatro exemplares apresentaram mais do que 10 metacercárias em seus olhos. Não foram observadas correlação entre abundância média e comprimento padrão, peso total e fator de condição (p>0,05). Esse é o primeiro registro dessa espécie de metacercária para S. intermedius.

ABSTRACT This article presents a case of tick infestation of the lower eyelid by a previously unreported species. A 71-year-old male presented with a tick attached to the lower eyelid. The tick was identified morphologically, and then molecularly via polymerase chain reaction (PCR) and sequencing of its DNA. In addition, a review of the literature relevant to the genera of ticks associated with infestation of the human eye is provided. The tick, which was in the nymphal developmental stage, was first identified according to taxonomic keys as Dermacentor sp. For complete species identification, 16s rDNA gene PCR and sequencing were performed, which showed that the tick was D. marginatus. Systematizing tick species could assist physicians in determining the potential for transmission of tick-borne human diseases.


RESUMO Este artigo apresenta um caso de infestação por carrapatos da pálpebra inferior por uma espécie previamente não declarada. Um homem de 71 anos de idade apresentou-se com um carrapato grudado na pálpebra inferior. O carrapato foi identificado morfologicamente, e, em seguida, uma estrutura molecular através de reacção em cadeia da polimerase (PCR) e a sequenciação do seu DNA. Além disso, uma análise da literatura pertinente aos gêneros de carrapatos associados à infestação do olho humano é fornecido. O carrapato, que estava em fase de desenvolvimento das ninfas, foi identificado pela primeira vez de acordo com chaves taxonômicas com o Dermacentor sp. Para identificação de espécies completa, gene 16S rDNA PCR e sequenciamento foram realizadas, que mostrou que o carrapato foi D. marginatus. Sistematizando espécie de carrapato poderia ajudar os médicos a determinar o potencial de transmissão de doenças humanas transmitidas por carrapatos.

Introducción. Toxocara canis es un nematodo patógeno de cánidos que accidentalmente puede ser transmitido a los humanos. A pesar de la importancia de la serología para el diagnóstico de esta zoonosis, los kits diagnósticos usan antígenos crudos de excreción-secreción, en su mayoría glucoproteínas que no son específicas de especie, por lo cual pueden presentarse reacciones cruzadas con anticuerpos generados contra otros parásitos. Objetivos. Producir el antígeno recombinante TES-30 de T. canis y evaluarlo para el inmunodiagnóstico de la toxocariasis. Materiales y métodos. Se clonó el gen que codifica TES-30 en el vector de expresión pET28a (+), usando oligonucleótidos de cadena sencilla unidos mediante reacción en cadena de la polimerasa (PCR). La proteína rTES-30 se purificó por cromotografia de afinidad (Ni 2+ ). La reacción serológica de rTES-30 se evaluó mediante immunoblot . Teniendo en cuenta que no existe una prueba de referencia , se observó el comportamiento del antigeno en comparación con la prueba de rutina para el inmunodiagnóstico de la toxocariasis, es decir, la técnica ELISA convencional con antígenos de excreción-secreción. Resultados. El rTES-30 se produjo a partir de un cultivo de Escherichia coli LB, con un rendimiento de 2,25 mg/l y 95 % de pureza. La concordancia de la reacción entre el immunoblot rTES-30 y la ELISA convencional, fue de 73 % (46/63) y de 100 % con los 21 sueros no reactivos. De los 21 sueros con diagnóstico de otras parasitosis, 19 fueron reactivos con ELISA, mientras que tan solo siete fueron positivos con el immunoblot rTES-30. La concordancia entre la ELISA y el immunoblot fue moderada (índice kappa de 0,575; IC 95% 0,41-0,74). Conclusiones. Los datos presentados respaldan la utilidad del immunoblot r TES-3 0 para la confirmación de los posibles positivos por ELISA, no solo en los estudios epidemiológicos, sino también, como candidato para el desarrollo de pruebas diagnósticas de la toxocariasis ocular en Colombia.


Introduction: Toxocara canis is a pathogenic nematode of canines which can be accidentally transmitted to humans. Although serology is the most important diagnostic tool for this zoonosis, diagnostic kits use crude excretion/secretion antigens, most of them being glycoproteins which are not species-specific and may cross-react with antibodies generated against other parasites. Objectives: To produce the rTES-30 recombinant antigen of Toxocara canis and evaluate it in the immunodiagnosis of toxocariasis. Materials and methods: The gene that codes for TES-30 was cloned in the expression vector pET28a (+) using single-stranded oligonucleotides united by PCR. The protein rTES-30 was purified by Ni 2+ affinity chromotography. Seroreactivity of rTES-30 was evaluated by immunoblot. Given that there is no gold standard test, the behaviour of the antigen was compared with the method that is routinely used to immunodiagnose toxocariasis, i.e., the conventional ELISA technique using excretion/secretion antigens. Results: The rTES-30 was produced from an Escherichia coli LB culture which yielded 2.25 mg/L of the antigen with a purity of 95%. The results obtained showed 73% (46/63) concordance of reactivity between the rTES-30 immunoblot and the conventional ELISA, and 100% concordance with the non-reactive sera (21). Nineteen of the 21 sera positive for other parasitoses reacted with ELISA, while only seven of these were positive with the rTES-30 immunoblot. Concordance between the ELISA and the immunoblot was moderate (kappa coefficient: 0.575; 95% CI: 0.41- 0.74). Conclusions: The data presented show the potential of the rTES-30 inmunoblot for confirmation of possible ELISA positives, not only in epidemiological studies, but also as a candidate for the development of diagnostic tests for ocular toxocariasis in Colombia.

RESUMO A miíase é a infestação dos tecidos humanos por larvas Diptera. O comprometimento ocular é raro. Os autores apresentam um caso de miíase na topografia do saco lacrimal e discutem as modalidades terapêuticas para o tratamento desta doença.


ABSTRACT Myiasis is the invasion of human tissues by Diptera larvae. Ocular involvement is rare. The objective of this paper is to report a case of myiasis in the topography of the lacrimal sac and discuss current treatments for this disease.

PURPOSE: This study evaluated the prevalence of ocular toxocariasis (OT) in patients with uveitis of unknown etiology who visited a tertiary hospital in South Korea and assessed the success of serum anti-Toxocara immunoglobulin G (IgG) enzyme-linked immunosorbent assay (ELISA) as a diagnostic test for OT. METHODS: The records of consecutive patients with intraocular inflammation of unknown etiology were reviewed. All participants underwent clinical and laboratory investigations, including ELISA for serum anti-Toxocara IgG. OT was diagnosed based on typical clinical findings. Clinical characteristics, seropositivity, and IgG titers were compared between patients diagnosed with OT and non-OT uveitis. The seropositivity and the diagnostic value of anti-Toxocara IgG was investigated among patients with different types of uveitis. RESULTS: Of 238 patients with uveitis of unknown etiology, 71 (29.8%) were diagnosed with OT, and 80 (33.6%) had positive ELISA results for serum anti-Toxocara IgG. The sensitivity and specificity of the ELISA test were 91.5% (65 / 71) and 91.0% (152 / 167), respectively. The positive predictive value of the serum anti-Toxocara IgG assay was 81.3%. Among patients with anterior, intermediate, posterior, and panuveitis, the prevalence rates of OT were 8.3%, 47.1%, 44.8%, and 7.1%, respectively; the seropositivity percentages were 18.1%, 47.1%, 43.7%, and 17.9%; and the positive predictive values were 38.5%, 95.8%, 92.1%, and 40.0%. The serum anti-Toxocara IgG titer also significantly decreased following albendazole treatment. CONCLUSIONS: OT is a common cause of intraocular inflammation in the tertiary hospital setting. Considering that OT is more prevalent in intermediate and posterior uveitis, and that the positive predictive value of the anti-Toxocara IgG assay is high, a routine test for anti-Toxocara IgG might be necessary for Korean patients with intermediate and posterior uveitis.

ABSTRACT

ABSTRACT Visceral Leishmaniasis, also know as Kala-azar, is a parasitic tropical disease caused by protozoa of the genus Leishmania donovani. It is an endemic disease in many countries. It affects approximately 1,5 million people every year, and when associated with mal-nutrition and co-infection it may be fatal. Fever, hepatosplenomegaly, and pancytopenia is its typical clinical picture. Ocular manifestations of Kalaazar are relatively rare and can affect either anterior or posterior segment of the eye. We report a patient with kala-azar presenting intraretinal hemorrhages that regress completely after the successful treatment for visceral leishmaniasis.


RESUMO Leishmaniose visceral, também conhecida como calazar é uma doença tropical parasitária, causada pelo protozoário do gênero Leishmania donovan uma doença endêmica em muitos países. Afeta aproximadamente 1,5 milhões de pessoas durante todo ano e quando associada à desnutrição e coinfecção pode ser fatal. Febre, hepatoesplenomegalia e pancitopenia e o quadro típico. Manifestações oculares são raras e podem afetar tanto o segmento anterior como o posterior do olho. Relatamos o caso de um paciente com calazar e hemorragia intrarretiniana que regrediu após tratamento para leishmaniose visceral.

Objetivo: Analizar la epidemiología, los métodos diagnósticos y los abordajes terapéuticos de la toxocariasis ocular en el Hospital de Pediatría Juan P. Garrahan. Materiales y métodos: Se realizó un estudio observacional y descriptivo en el Servicio de Oftalmología del Hospital de Pediatría J. P. Garrahan analizándose en forma retrospectiva los pacientes con ELISA positivo para IgG anti-Toxocara canis vistos en el laboratorio de Parasitología y evaluados en el servicio de Oftalmología entre enero 2006 a junio de 2013. Resultados: Se incluyeron en el estudio 301 pacientes de los cuales 95 (31,5%), presentaron diagnóstico de toxocariasis ocular (59 niños y 36 niñas) y 206 no tuvieron afección oftálmica por Toxocara canis. El 100% de los pacientes con afección ocular por el parásito presentó algún grado de alteración de la visión. Las formas de toxocariasis ocular que se registraron son: granuloma periférico aislado en 12 pacientes (12,6%), granuloma periférico con pliegue unido a papila en 36 (37,9%), granuloma de polo posterior en 7 (7,4%), endoftalmitis crónica en 3 (3,2%), neurorretinitis subaguda unilateral difusa en 2 (2,1%), desprendimiento de retina en 23 (24,2%) y en 12 (12,6%) forma indeterminada. Conclusiones: El diagnóstico de la toxocariasis ocular en la infancia tiende a hacerse en forma tardía ya que los niños pequeños no suelen manifestar a los padres la disminución visual de un ojo siendo el motivo de consulta más frecuente el estrabismo. El diagnóstico es clínico. La forma oftalmoscópica de presentación más frecuente en nuestro estudio fue el granuloma periférico con pliegue falciforme unido a papila. Oftalmoscopicamente la toxocariasis ocular puede presentarse de formas sumamente disímiles, lo cual hace dificultoso su diagnóstico y obliga a pensar siempre en éste en un niño que presenta inflamación intraocular unilateral. El uso de antiparasitarios en nuestros pacientes fue irregular (AU)


Objective: To assess the epidemiology, diagnostic methods, and treatment approach in ocular toxocariasis at the Pediatric Hospital Juan P. Garrahan. Material and methods: An observational descriptive study was conducted at the Department of Ophthalmology of the Pediatric Hospital Juan P. Garrahan. Patients with a positive ELISA for anti-Toxocara canis IgG seen at the laboratory of parasitology and evaluated at the Department of Ophthalmology between January 2006 and June 2013 were retrospectively analyzed. Results: 301 patients were included in the study of whom 95 (31.5%) had a diagnosis of ocular toxocariasis (59 boys and 36 girls) and in 206 the eyes were not affected by toxocariasis. All patients with ocular toxocariasis had some degree of visual impairment. The forms of ocular toxocariasis found were: isolated peripheral granuloma in 12 patients (12.6%), peripheral granuloma with the fold united to the papilla in 36 (37.9%), posterior pole granuloma in 7 (7.4%), chronic endophthalmitis 3 (3.2%), diffuse unilateral subacute neuroretinitis 2 (2.1%), retinal detachment in 23 (24.2%), and an indeterminate form in 12 (12.6%). Conclusions: The diagnosis of ocular toxocariasis in childhood is often late as children are not able to report vision loss to their parents. Strabism is the most common reason for consult. The diagnosis is clinical. The most frequent ophthalmoscopic presentation was peripheral granuloma with a falciform fold attached to the papilla. Ophthalmoscopically, ocular toxocariasis may present heterogeneously, resulting in a difficult diagnosis. The disease should always be suspected in a child with unilateral intraocular inflammation. The use of antiparasitic agents was variable in our patients (AU)

Background: Blepharitis is a very common disease in the ophthalmologic practice generally taking a chronic course with intermittent exacerbations. Several studies have linked the presence of Demodex folliculorum with chronic blepharitis, since the mite has the capacity to perpetuate the follicular inflammatory process. The prevalence of infection by Demodex spp. is variable depending on the population. In Paraguay, information on the frequency of the infestation in patients with chronic blepharitis is not available. Aim : To determine the frequency of Demodex spp, and the ocular microbiota in patients with chronic blepharitis attending the Department of Ophthalmology at the Teaching Hospital of the National University of Asuncion. Patients and Methods: Consecutively, 28 patients with chronic blepharitis, who agreed to participate in the study, were included. Eyes lashes from the upper and lower eyelids were extracted for immediate mite search by direct observation under a light microscope. Samples from eyelids were taken with Kimura spatula and then cultured on blood agar and in enrichment media and incubated in 5% CO2 at 35° C for 72 hours. Results: Among participants, females were more frequent (64%), the age ranged from 17 to 87 years (mean: 38.0; SD: ±13.5 years). The prevalence of Demodex sp was 54%. Bacteria were isolated 92.9% of cases, most frequently coagulase-negative staphylococci (75%). No association was found between socio-demographic or clinical characteristics and the presence of Demodex sp. Conclusion: The observed high prevalence of infestation by Demodex spp in patients with chronic blepharitis is consistent with other studies.


Introducción: La blefaritis es una enfermedad muy común en la práctica oftalmológica, generalmente de curso crónico con exacerbaciones intermitentes. Varios estudios han relacionado la presencia del Demodex folliculorum con la blefaritis crónica, por su capacidad potencial de perpetuar el proceso inflamatorio a nivel folicular. La frecuencia de D. folliculorum varía de acuerdo a la población. Objetivo: Determinar la presencia de Demodex spp y, caracterizar la microbiota ocular en pacientes con blefaritis crónica y dar a conocer estos resultados. Pacientes y Métodos: Las muestras fueron obtenidas de pacientes que consultaron en la Cátedra de Oftalmología del Hospital de Clínicas de la Universidad Nacional de Asunción (UNA). Fueron incluidos en forma consecutiva 28 pacientes con blefaritis crónica que accedieron a participar en forma voluntaria en el estudio. Se extrajeron las pestañas del párpado superior e inferior de los pacientes para la búsqueda inmediata del ácaro por observación directa bajo el microscopio óptico. Para el cultivo de microorganismos se tomaron muestras del párpado con espátula de Kimura, las que fueron cultivadas en agar sangre y en medios de enriquecimiento e incubadas en CO2 al 5% a 35°C durante 72 h. Resultados: El sexo femenino fue más frecuente (64%), el rango de edad estuvo entre 17 y 87 años con una media de 37,9 ± 13,5. Se observó la presencia de Demodex sp en 54% y aislamiento de bacterias en 92,9%; Staphylococcus coagulasa negativa fue el más frecuente (75%). No se encontraron asociaciones entre las características socio-demográficas o clínicas y la presencia de Demodex sp. Conclusión: La alta infestación por Demodex spp observada en los pacientes con blefaritis crónica coincide con otros estudios.

ABSTRACT

A 52-yr-old male was referred for progressive visual loss in the left eye. The decimal best-corrected visual acuity (BCVA) was 0.01. Fundus examination revealed diffuse retinal pigment epithelial degeneration, focal yellow-white, infiltrative subretinal lesion with fuzzy border and a live nematode within the retina. Diffuse unilateral subacute neuroretinitis (DUSN) was diagnosed and the direct laser photocoagulation was performed to destroy the live nematode. During eight months after treatment, BCVA gradually improved to 0.2 along with the gradual restoration of outer retinal layers on SD-OCT. We report on the first case of DUSN in Korea. DUSN should be included in the differential diagnosis of unexplained unilateral visual loss in otherwise healthy subjects.

ABSTRACT

New occurrences and effects of parasitism by metacercariae in fish eyes have recently been discussed in many studies. The aim of the present study was to evaluate the infection levels of the eye flukes Sphincterodiplostomum musculosum Dubois, 1936 (metacercariae) in Steindachnerina insculpta (Fernádez-Yépez, 1948) from three ecosystems under the influence of the Jurumirim reservoir (Paranapanema, Taquari and Veados Rivers). A total of 174 fish specimens were sampled between April 2011 and April 2012. There were high levels of infection by S. musculosum metacercariae in the eyes of S. insculpta from all the sampled ecosystems, thus presenting broad geographical distribution. The histological analyses revealed that the metacercariae were apparently lodged behind the retina. The smaller fish from the Taquari River were less parasitized by metacercariae than the adults. This pattern can be explained by the cumulative effect of parasitism, as demonstrated by the positive correlation between the abundance of metacercariae and fish body size.


Novas ocorrências e efeitos do parasitismo por metacercárias em olhos de peixe têm sido recentemente discutidas em muitos estudos. O objetivo deste estudo foi avaliar os níveis de infecção dos vermes de olho Sphincterodiplostomum musculosum Dubois, 1936 (metacercárias) em Steindachnerina insculpta (Fernádez-Yépez, 1948) de três ecossistemas influenciados pelo reservatório de Jurumirim (Rios Paranapanema, Taquari e Veados). Um total de 174 espécimes de peixes foi amostrado de Abril/2011 a Abril/2012. Metacercárias de S. musculosum mostraram altos níveis de infecção nos olhos de S. insculpta em todos os ecossistemas amostrados apresentando ampla distribuição geográfica. As análises histológicas revelaram que, aparentemente, estas metacercárias estavam alojadas por trás da retina. Os peixes menores do rio Taquari estavam menos parasitados por metacercárias do que os adultos. Esse padrão pode ser explicado pelo efeito cumulativo de parasitismo, evidenciado pela correlação positiva entre a abundância de metacercárias e tamanho do corpo dos peixes.

ABSTRACT

11 years old girl, from south region of Chile, without history of travels outside Chile nor the province, complaints of red eye with blepharitis and blood-tingued epiphora. Eye exam revealed a pseudomembrane. Clinical diagnosis was folicular conjunctivitis. A surgical removal was performed and the lesion sent to biopsy analysis. On microscopic examination numerous 50-150 μm cysts with keratinous wall and numerous endospores were found. Rinosporidiosis is an infection caused by Rhinosporidium seeberi that frecuently affects nasal cavity but could infect eye, urogenital tract and airways. This infections is considered endemic in Asia and Africa, but it is very important to have the suspicious among polyps in these areas because travel to endemic areas is become more frecuently.


Escolar de 11 años, de sexo femenino, proveniente de la Región del Bío-Bío, sin antecedentes de viajes, consultó por ojo rojo, blefaritis y epífora sanguinolenta; al examen oftalmológico reveló una pseudomembrana. La sospecha clínica fue de una conjuntivitis folicular. Se realizó la remoción quirúrgica de la lesión y en el estudio histopatológico se observaron lesiones características de rinosporidiosis, correspondiente a quistes de 50-150 μm con pared quitinosa y numerosas endosporas. La rinosporidiosis es una infección causada por Rhinosporidium seeberi, que afecta con mayor frecuencia la cavidad nasal, pero puede también afectar el ojo, aparato urogenital y la vía aérea, entre otros. Es considerado endémico en países de Asia y África, pero dado el aumento de viajes a estas regiones es importante reconocer este agente como causal de pólipos en la zona nasal y ocular.

Introducción. La resistencia bacteriana es crítica para la selección de los antibióticos en el tratamiento de las infecciones, por ello es vital conocer su estado actual en nuestro medio. Objetivo. Determinar la sensibilidad antibiótica bacteriana in vitro obtenida de los cultivos de queratitis e infecciones intraoculares. Materiales y métodos. Se llevó a cabo un estudio retrospectivo en la Fundación Oftalmológica de Santander (FOSCAL), entre junio de 2011 y enero de 2012. Resultados. Se examinaron 92 muestras. Se identificaron 110 bacterias, 27 hongos y 12 amebas de vida libre. Del total de bacterias Gram positivas, 1,1 %, 0 %, 1,1 %, 16,9 %, 29,3 % y 85 % fue resistente a imipenem, moxifloxacina, gatifloxacina, levofloxacina, ciprofloxacina y tobramicina, respectivamente, mientras que la resistencia a estos mismos fármacos se presentó, respectivamente, en 0 %, 8,3 %, 0 %, 0 %, 18,2 % y 27,3 % de las bacterias Gram negativas. Los porcentajes de resistencia de los estafilococos positivos para coagulasa resistentes a la meticilina fueron 0 %, 0 %, 0 %, 7 %, 17 % y 100 %, respectivamente, y los porcentajes de los estafilococos negativos para coagulasa resistentes a la meticilina fueron 3 %, 0 %, 0 %, 24 %, 44 % y 100 %, respectivamente. Los porcentajes de resistencia bacteriana globales (tanto para bacterias Gram positivas como para Gram negativas) a imipenem, moxifloxacina, gatifloxacina, levofloxacina, ciprofloxacina y tobramicina fueron 1 %, 1 %, 1 %, 15,1 %, 28 % y 64,5 %, respectivamente. Conclusiones. Los niveles de resistencia bacteriana para imipenem, moxifloxacina y gatifloxacina fueron menores que para levofloxacina, ciprofloxacina y tobramicina. Los niveles de resistencia para la tobramicina fueron muy altos, lo que pone en duda su utilidad clínica en las infecciones oculares en nuestro medio.


Introduction: Bacterial resistance is critical for the selection of antibiotics in the treatment of infections, so it is vital to know its current status in our geographical area. Objective: To determine in vitro antibiotic susceptibility of bacterial isolates obtained from keratitis and intraocular infections. Materials and methods: A retrospective study of microbiological tests in Fundación Oftalmológica de Santander (FOSCAL) was carried out between June, 2011, and January, 2012. Results: A total of 92 samples were examined and 110 bacteria, 27 fungi and 12 free-living amoebae were identified. Polymicrobial infections constituted 50% of the total; 1.1%, 0%, 1.1%, 16.9%, 29.3% and 85% of Gram-positive bacteria were resistant to imipenem, moxifloxacin, gatifloxacin, levofloxacin, ciprofloxacin and tobramycin, respectively, while 0%, 8.3%, 0%, 0%, 18.2% and 27.3% of Gram-negative bacteria were resistant to imipenem, moxifloxacin, gatifloxacin, levofloxacin, ciprofloxacin and tobramycin, respectively. For methicillin-resistant coagulase-positive staphylococci, resistance percentages to imipenem, moxifloxacin, gatifloxacin, levofloxacin, ciprofloxacin and tobramycin were 0%, 0%, 0%, 7%, 17% and 100%, respectively. For methicillin-resistant coagulase-negative staphylococci, resistance percentages to imipenem, moxifloxacin, gatifloxacin, levofloxacin, ciprofloxacin and tobramycin were 3%, 0%, 0%, 24%, 44% and 100%, respectively. Overall bacterial resistance to imipenem, moxifloxacin, gatifloxacin, levofloxacin, ciprofloxacin and tobramycin, for both Gram-positive and Gram-negative, was 1%, 1%, 1%, 15.1%, 28% and 64.5%, respectively. Conclusions: The levels of bacterial resistance to imipenem, moxifloxacin and gatifloxacin were lower than for levofloxacin, ciprofloxacin and tobramycin. The levels of resistance to tobramycin were very high, which calls into question its usefulness in this region of our country.
